The relationship between oral health and autoimmune disorders has gained increasing attention in recent years due to the close interaction between the immune system and the oral cavity. Autoimmune diseases can affect various oral tissues, leading to complications such as dry mouth, oral ulcers, gum disease, and tooth decay. Conversely, chronic oral inflammation, particularly periodontal disease, may contribute to systemic inflammatory responses that can influence the severity of certain autoimmune conditions. Although poor oral health is not considered a direct cause of autoimmune diseases, maintaining good oral hygiene is an important part of comprehensive disease management. The oral cavity is one of the first parts of the body to reflect changes in overall health. Many systemic diseases, particularly autoimmune disorders, present with oral signs before affecting other organs. Since the immune system plays a crucial role in maintaining the health of the oral tissues, any disturbance in immune function can increase the risk of infections, inflammation, and tissue damage within the mouth. Likewise, chronic oral diseases such as periodontitis may contribute to systemic inflammation, potentially influencing autoimmune disease activity. Understanding this two-way relationship enables earlier recognition of oral symptoms, timely treatment, and better coordination between medical and dental care, ultimately improving both oral and overall health.

What is an autoimmune disorder?

An autoimmune disorder is a condition in which the body’s immune system mistakenly attacks its own healthy cells and tissues. This abnormal immune response causes inflammation and damage to different organs and tissues. There are many autoimmune disorders, each with different symptoms depending on the part of the body affected.

How do autoimmune disorders affect oral health?

  1. Damage to vessels and nerves – The surrounding and supporting structures of teeth start to damage. There is loss of sensations due to nerve damage in the mouth.
  2. Mouth ulcers -Disorders like Lupus and Crohn’s disease can lead to ulceration easily.
  3. Dental caries – As there is decrease in salivary flow , bacteria invasion increases. Autoimmune disorders attack the salivary glands.
  4. Temporomandibular joint (TMJ) disorders – Occurs when the joint of mandible (lower jaw) gets inflamed that leads to pain, and limited movement.
  5. Gum disease – Body’s natural defense mechanism lowers due to autoimmune disorders which makes the gums more prone to gingivitis.
  6. Diabetes – Due to increased glucose levels in blood, bacteria tend to thrive and can cause gum and periodontal diseases which further can cause autoimmune disorder.

1. Sjogren’s syndrome

It is a disease that affects the salivary glands and lacrimal glands reducing its activity, decreasing the salivary flow and lacrimal fluid. It affects minor and major salivary glands both.

Signs and symptoms

  1. Hyposalivation (decrease in salivary flow)
  2. Dry mouth ( xerostomia)
  3. Decreased tear flow ( xerophthalmia)

In its pathogenesis, environmental and genetic factors play a major role. Activity of B-cells increases forming an immune complex and production of antibodies takes place. This disease occurs more in women than men. It affects people of about 50 years of age. Lack of saliva can lead to dental cavities. Inflammation of gingiva also occurs.

2. Lichen planus

It is a disorder of the skin that leads to formation of rashes on an area of the body or many parts together at one time. It can cause inflammation in our skin. The immune cells attack other body parts. In such people, T-cells attack the skin cells and mucus membranes but cause of this attack is idiopathic.

Signs and symptoms

  1. Little, raised dots or papules on skin or even the genitals.
  2. White dots on cheeks, lips or tongue
  3. Nails become brittle, cracked, color changes occur
  4. In severe cases, sores can form.

In people with lichen planus, scratching may lead to rashes or breaking of skin which can further cause infections and pain.

3. Pemphigus vulgaris

It is a rare autoimmune condition where the body’s immune cells attack a protein on the skin cells.

Signs and symptoms

  1. Blisters on skin
  2. Sores in mouth
  3. Nikolsky sign ( surface layers of skin shed off easily on rubbing)
  4. Erosions on mucus membranes or skin

This disorder may affect the eyes, throat, genitals, mouth and nose. It commonly affects people of middle age around 40 and above.

4. Type 1 Diabetes

It is a specific autoimmune condition in which the body’s immune cells damage insulin producing cells of pancreas (beta cells). It results in insulin deficiency.

Signs and symptoms

  1. Urge to urinate increases
  2. Thirst increases
  3. Tiredness
  4. Weight loss

If Type 1 diabetes is not managed, it may cause diabetic ketoacidosis. Majorly adults are affected.

5. Systemic Lupus Erythematosus(SLE)

SLE is a chronic autoimmune disorder that is inflammatory in nature. Dysfunctioning of the immune system leads to the production of autoantibodies.

Signs and symptoms

  1. Butterfly rash (a raised, red rash spreading from nose bridge and cheeks)
  2. Sensitivity to sun
  3. Pain and swelling in joints
  4. Pain in chest
  5. Fatigue and extreme tiredness

It can lead to increased mortality prematurely. There is an increase in autoantibodies in the body. It affects multiple organs- CNS, skin, kidney.

6. Scleroderma

It is a rare disorder in hich there is growth of connective abnormally and excess collagen production.

Signs and symptoms

  1. Hard , shiny patches on skin
  2. Pain in muscles
  3. Gastrointestinal issues (difficulty in swallowing, heartburn,bloating)
  4. Raynaud phenomenon (poor circulation in digits ,cold sensitivity)

Prevention measures for a good oral health

  1. Brushing twice a day for at least two minutes.
  2. Cleaning the tongue properly.
  3. Use of soft-bristled toothbrush and fluoridated toothpaste.
  4. Flossing of teeth daily with water flosser or basic floss.
  5. Incorporate a balanced diet in your routine.
  6. Limit use of sugary and starchy food and drinks.
  7. Quit smoking and tobacco
  8. Avoid excessive alcohol consumption.
  9. Visit for dental check ups routinely.

When our immune system starts attacking the body’s healthy cells it causes autoimmunity in the body. There are various factors that lead to its progression like genetics, environmental factors, chemical factors,drugs, diet or any existing infection.
