SCA12 is inherited in an autosomal dominant manner, meaning that a mutation in one copy of the responsible gene is sufficient to transmit the disorder to offspring. The disease develops due to abnormal expansion of CAG trinucleotide repeats within the PPP2R2B (Protein Phosphatase 2 Regulatory Subunit B Beta) gene. This mutation alters neuronal function and eventually causes degeneration of cerebellar Purkinje cells and related neural circuits. Unlike several other spinocerebellar ataxias that begin predominantly with gait imbalance, SCA12 commonly presents with action tremors, especially involving the upper limbs. Over time, affected individuals gradually develop cerebellar ataxia, dysarthria, intention tremor, impaired handwriting, gait instability, and difficulties in performing fine motor activities. Reference: Charaka Samhita, Sharira Sthana 2/29–30 These verses explain that abnormalities in the Beeja (reproductive seed), the influence of Karma (deeds), the appropriate timing of conception, and maternal dietary and lifestyle factors can collectively produce congenital deformities and functional abnormalities in the developing fetus. This principle closely resembles the inherited genetic disorders. In SCA12, the mutation in the PPP2R2B gene can be correlated with Beeja Bhagavayava Dushti, wherein only a specific component of the Beeja responsible for the normal development and function of the nervous system becomes defective. Since the pathology predominantly affects neuronal tissues controlling voluntary movement and coordination, it can be interpreted as Majjavaha Beeja Bhagavayava Dushti. Furthermore, Ayurveda acknowledges that certain diseases arise despite proper conduct and healthy living due to Purvajanma Krita Karma, and such conditions are described under Karmaja or Doshakarmaja Vyadhi. Although this concept is philosophical rather than genetic, it recognizes that some disorders originate from factors beyond those immediately observable, thereby complementing the hereditary nature of diseases such as SCA12. From the viewpoint of Dosha involvement, SCA12 exhibits the characteristic features of Vatavyadhi, as Vata Dosha governs all voluntary and involuntary movements, neuromuscular coordination, sensory perception, speech, and higher neurological functions. The progressive degeneration of cerebellar pathways produces tremors, dysarthria, gait instability, impaired balance, and incoordination, indicating aggravation of Vata dosha affecting the Majjavaha Srotas. Continuous Vata dosha aggravation eventually leads to Majja Dhatu Kshaya (depletion), resulting in depletion of nervous tissue and progressive neurological deterioration. Thus, the Ayurvedic pathogenesis (Samprapti) of SCA12 can be understood as an inherited Beeja or Beeja Bhagavayava Dushti that predisposes an individual to neurological degeneration, followed by progressive aggravation of Vata Dosha, localization in the Majjavaha Srotas, depletion of Majja Dhatu, and gradual manifestation of tremors, ataxia, speech impairment, and gait disturbances.
