ARTHROGRYPOSIS MULTIPLEX CONGENITA

ABSTRACT

Arthrogryposis Multiplex Congenita or AMC is a rare congenital disease affecting the body physically in various ways. In fact, it is known to be a physical sign that is expressed in various diseases. One or more areas of the body get affected by this where to occur contractures that are non-progressive in nature.  It is congenital in nature which means present by birth.

INTRODUCTION

Arthrogryposis multiplex congenita causes fixing of the joints leaving them flexed, i.e. fixed in a bent position or extended, i.e. in a straight position. This disorder causes partial or complete fixing of the joints resulting in restriction of the movements. The peculiar feature of  Arthrogryposis multiplex congenita is the presence of congenital contractures in various parts of the body which differentiate it from isolated congenital contracture where the contracture or fixation is in a single part of the body. Clubfoot is an example of AMC.

As it is a congenital disease so the signs and symptoms are present since birth. There can be seen a wide range of variation in signs and symptoms from patient to patient even if are from the same family. This difference can be in the severity and locality. After birth infant shows contractures in various joints like elbows, shoulder, ankles, wrists, hips, fingers, and toes. Usually, the leg joints are affected as compared to the joints of the arms. Arthrogryposis can even affect the back and jaw of the patient and causes contractures there. Mutations in certain genes are known to be responsible for this condition.

SIGNS AND SYMPTOMS

• Fixed or limited movements of various joints in the body
• Small and large any joint can be affected
• These contractures are seen since birth in an infant
• Underdevelopment of the muscles of the affected joint which is termed as hypoplasia
• Limbs feel soft and doughy
• Bones appear to be large, slender and fragile
• Cleft palate is common in these patients
• Testes fail to descends in males
• Sometimes intellect if affected in such patients
• Central nervous systems have functional and structural abnormalities

CAUSES

The cause of Arthrogryposis multiplex congenita is usually not known in many cases. However its etiology is found to be consisting of many factors. One of the causes which are known for decreased movement of the fetus in the uterus and this decreased movement can be due to many reasons like a defect in muscles and nervous system, illness of the mother, disorders of connective tissues, and space limitation in the uterus.

AMC is also a genetic condition that occurs due to the abnormality in a certain gene. This gene is passed from either of the parents, being a carrier of the defected gene. Improper development of the nervous system, connective tissues, or peripheral nervous system can lead to the causation of AMC.

DIAGNOSIS

The presence of certain characteristic symptoms leads to suspecting Arthrogryposis multiplex congenita. Through the history of the patient, physical examination and nerve conduction tests are a source of confirmation. Muscle biopsy and Electromyography also helps diagnosis of AMC.

TREATMENT

Arthrogryposis multiplex congenita is treated symptomatically. Usually the mode of treatment used in AMC is physical therapy to improve the movement of the joints, and prevent the atrophy of muscles. In some severe cases even surgeries are advised to enhance the movement of joints particularly the shoulfder, elbow, hip, knee and wrist joints. Tendon transfer is also a technique used to treat AMC.
