Herbal Remedies for Meralgia Paresthetica (Bernhardt-Roth Syndrome)

ABSTRACT

The nervous system is the commanding system of the body. It is responsible for the appropriate functioning of the various bodily mechanisms. The nervous system constitutes a network of numerous nerves in the body. The nerves are responsible for carrying or transmitting signals between the brain and other parts of the body. This mechanism is responsible for touch sensation, reflexes, various voluntary and involuntary functions, etc. What happens if there is an abnormality in this mechanism? Or if the nerves of some region are not functioning properly or are damaged? There can be various conditions in which there is abnormality or damage in the nerves, hindering the normal physiology of the body. One such related condition is the Meralgia Paresthetica (bernhardt-roth syndrome), which will be described in detail along with its description of symptoms, causes, ayurvedic aspect and management in the following article.

Symptoms of Meralgia Paresthetica (Bernhardt-Roth Syndrome)

INTRODUCTION

Meralgia paresthetica (bernhardt-roth syndrome) is a condition that is a result of either excessive pressure or damage to one of the essential nerves of the leg namely lateral femoral subcutaneous nerve. It usually affects the upper outer thigh region and is characterized by severe and often painful paresthesia in the area of the upper outer thigh. Paresthesia can be explained as a type of pain, prickling in nature. Meralgia paresthetica affects approximately 1-4% of the general population. The actual condition is less common than the term meralgia paresthetica since it implies a specific diagnosis. However, there are many other causes of lateral thigh pain besides meralgia paresthetica. The causes resulting in the condition of Meralgia paresthetica (bernhardt-roth syndrome) will be discussed as follows.

CAUSES

The causes of the condition of Meralgia paresthetica (bernhardt-roth syndrome) can be various; some of the most common are being explained ahead. Excessive pressure on the lateral femoral nerve from excessive muscle activity which causes mechanical injury to this nerve can cause meralgia paresthetica. In such a case it can be caused by association with traction of the nerve as it passes under the hip joint. This causes mechanical injury to this nerve that leads to a cutaneous innervation of the skin. As fibrous tissue forms on this affected area, the superficial vessels become occluded and the superficial nerves are irritated causing numbness and pain in the form of meralgia paresthetica. It is more common in females than males after age 30 years as well as in diabetics. It may be associated with anemia, femoral neuropathy, spine fractures and arthritis.

SYMPTOMS

Meralgia paresthetica (bernhardt-roth syndrome) is basically characterized by pain in the thigh region, more specifically the outer thigh region. This pain can be accompanied with various other features also. There can be a tingling sensation along with the pain. The patient may complain of a burning sensation on the outside of the thigh. There can be high sensitivity to heat and light touch. The pain can be observed to rise after walking or standing for a long time. The patient may feel mild degree pain in the initial stages but as the condition progresses, he/ she might experience increasing intensity. The pain may get sharper and more of shooting nature.

DIAGNOSIS & TREATMENT

The diagnosis of Meralgia paresthetica (bernhardt-roth syndrome) will include various types of examinations. The tests advised will include imaging tests such as X-rays, CT scan, etc. Electromyography, nerve conduction test and blood tests may also be required. After the diagnosis is done, the next step should be management of the condition. The management will include lifestyle changes such as weight loss, wear loose fit clothes, etc. and physical therapy may include ultrasound therapy through skin, or transcutaneous electrical nerve stimulation, etc.

AYURVEDIC ASPECT

Ayurveda is an ancient old science which has not lost its relevance even in these modern times. Modern science explains Meralgia paresthetica (bernhardt-roth syndrome) as an abnormality related to the nerves, Ayurvedic view point varies. Ayurveda presents a very unique perspective on the various health related issues. It supports the body to get rid of the disease by establishing the equilibrium between the three dosha (vata, pitta, kapha). These three dosha are considered to be the fundamentals of a healthy body. And an imbalance between the levels of these dosha will result in the diseased state. The condition of Meralgia paresthetica (bernhardt-roth syndrome) is also a result of the imbalance between the equilibrium of these dosha.

In every disease, all the three dosha are vitiated but one or two of them are more prominently hindered, and constitute the major presenting features. In the case of Meralgia paresthetica (bernhardt-roth syndrome), the major dushti or vitiation is believed to be of the vata dosha. The patient experiences prickling pain which is similar to the characteristic Suchibhedan vata peeda (i.e. pain similar to that of being pricked by a needle) presented by the vitiation of vata dosha. There is numbness also in some cases which can be related to suptata in ayurveda which is also a result of vata vitiation. Then, apart from vata, mild vitiation of pitta can also be considered as the patient experiences burning sensation also. The daha or burning sensation experienced in the body is a result of the vitiated pitta dosha in the body. So, the main line of ayurvedic management of the condition of Meralgia paresthetica (bernhardt-roth syndrome) will require balancing the levels of vata and pitta dosha. This can be accomplished by the administration of various ayurvedic formulations such as ashwagandha capsules, boswellia curcumin, hakam churna, etc. These ayurvedic preparations will help to manage the diseased condition and enhance the health of the body.
