Sialadenitis or Sialadenosis- Causes Symptoms and Herbal Treatment

ABSTRACT

In normal terms, Sialadenitis is inflammation of the salivary glands whereas Sialadenosis is non-inflammatory swelling of salivary glands. The salivary gland helps to make saliva and saliva helps in swallowing and mixing of food and protects the teeth from infection. There are three salivary glands present in our mouth: parotid gland, sub mandibular gland, and sublingual gland. Sialadenitis is due to infection, autoimmune disease, and salivary gland stones. Sialadenitis can affect any age group but is more common in adults more than 50 years old and especially affects the person who is affected by salivary gland stones. Staphylococcus aureus is the main cause of sialadenitis. If left untreated it can go to deep tissues of the neck and head and cause a severe infection. Sialadenosis is a non-inflammatory condition in which swelling primarily affects the parotid gland and sometimes affects the submandibular gland. It is mostly affected due to systemic metabolic disease.

Sialadenitis and Sialadenosis

INTRODUCTION

As we know Sialadenitis is inflammation of the salivary gland so due to infection it may spread from one person to another person by sneezing, or coughing, or when it is due to salivary stones it does not spread from one person to another, whereas Sialadenosis is associated with ductal atrophy and acinar hypertrophy. It is non-tender swelling and mostly symmetrical and bilateral. It is bilateral, and chronic, and commonly affects the parotid gland.

CAUSES

The causes of Sialadenitis are

  1. Dehydration
  2. Immunosuppressants patient  
  3. Malnutrition
  4. Poor oral hygiene
  5. Medications
  6. Virus- Mumps, HIV
  7. Ductal stricture
  8. External constriction of duct
  9. Sialoliathasis
  10.  Ductal foreign body

The causes of Sialoadenosis are-

  1. Vitamin deficiency
  2. Obesity
  3. Diabetes mellitus
  4. Liver cirrhosis
  5. Hypothyroidism
  6. Malabsorption

SYMPTOMS

The symptoms of Sialadenitis are

  1. Enlargement of salivary gland
  2. Tenderness of the salivary gland
  3. Discoloration of salivary gland
  4. Difficulty in eating
  5. Fever
  6. Chills
  7. Pus in the mouth
  8. Dry mouth
  9. Dry eye
  10. Mouth pain
  11.  The amount of saliva decreases
  12.  Inflammation in the neck and cheek region
  13.  A foul taste in the mouth
  14.  Weight loss

The symptoms of sialadenosis are

  1. Painless, soft, and bilateral enlargement of the parotid gland
  2. Dryness in mouth
  3. Mild discomfort
  4. The amount of saliva decreases
  5. Tenderness of glands

DIAGNOSIS OF SIALADENITIS AND SIALADENOSIS

The physician checks the patient and the following tests are done

  1. X-ray- To detect the sialolith
  2. USG- To detect sialolith(less than 1 cm), and abscess cavity
  3. CT (Computerised Tomography) Scan – To detect sialolith, in chronic cases salivary glands are enlarged or atrophic
  4. MRI (Magnetic resonance imagining)- To suspect malignancy
  5. FNAC of the affected gland- For the presence of a tumour
  6. DSA Sialography- To check sialolith, ductal stricture

TREATMENT

  1. Warm massage on the affected side
  2. Maintain oral hygiene
  3. Do massage of salivary gland
  4. Keep the patient hydrated
  5. Surgical removal of the gland

AYURVEDA ASPECT FOR SIALADENITIS AND SIALADENOSIS

As Acharya Sushrat सु.नि. १३।१३ explained पाषाणगर्दभ (disease of salivary glands). Due to the vitiation of kapha and vata dosha, there is shopha (edema) in the हनुसंधि (temporomandibular joints) termed as पाषाणगर्दभ. As Acharya explains 65 diseases of the oral cavity. Where 8 are the औष्ठगत रोग (disease of lips), 15 are the दन्तमूलगत रोग (disease of periodontia), 8 are दन्तगत रोग (dental disease), 5 are जिहवागत रोग (disease of tongue), 9 are तालुगत रोग (disease of palate), 17 are कण्ठगत रोग (disease of pharynx and larynx), and 3 are सर्वसर रोग(generalised mucosal affections of the oral cavity). The three doshas are the main elements of our body and are responsible for the normal functioning of the body. Each dosha plays an important role in the buccal cavity. As vata dosha is present in teeth pores, pitta dosha contains blood vessels that protect and nourish the teeth, and kapha dosha has a subtype that is bodhak kapha which lubricates the mucous membrane of the buccal cavity. Due to improper diet and improper activities, the dosha Parkopa (vitiates) will be here and result in a dosha imbalance. Due to dosha imbalance, there is inflammation and swelling of the glands.

1. Curcumin Capsules

It is made up of an extract of Haridra (Curcuma longa). It contains anti-cancerous properties, helpful in avoiding the formation of tumours. As curcumin has anti-inflammatory properties it reduces the inflammation of salivary glands.

Dosage– 1 capsule twice daily, with plain water, after meals.

2. Septrin tablets

Septrin tablets are made up of Tulsi (Ocimum sanctum), Haldi (Curcuma longa), Guggul (Commiphora mukul), and many more ingredients. It balances the three main energies of the body that is vata, pitta, and kapha dosha. Tulsi has properties of strengthening the gums and teeth which is helpful in treating mouth ulcers. 3. Kaishore Guggul

The tablet contains Red physic nut (Baliospermum montanum), Amalaki (Emblica officinalis), Indian Jalap (Ipomoea turpethum), Bibhitaki (Terminalia bellirica), Haritaki (Terminalia chebula), Guggul (Commiphora mukul) and many other herbs. As it contains triphala it decreases the formation of plaque in the oral cavity and maintains oral hygiene. Guduchi helps decrease the formation of plaques, prevents inflammation of gums, and removes bacteria from the oral cavity. Ginger helps to remove plaques, prevent cavities, and strengthen the gums.

Dosage – 2 tablets twice daily with lukewarm water.

4. Digestion support

Dosage– Massage teeth and gums with 3-6 gm powder. Rinse the mouth with lukewarm water after 2-3 minutes.
