Abstract

Nephrotic syndrome is characterised by hypoalbuminemia, peripheral edema, severe proteinuria and frequently in conjunction with hyperlipidemia and shows the symptoms such as swelling (edema) around the ankles, feets, eyes and foamy urine and various other. Introduction

Nephrotic syndrome is a kidney disorder that occurs when the kidney functions get hampered and start leaking excessive amounts of protein into the urine. The nephrotic syndrome contains the following findings such as massive proteinuria loss of 3.5 gm on daily bases or more, hypoalbuminemia with levels of plasma albumin less than 3 gm per dl, edema mainly around ankle, eyes and feet and hyperlipidemia. This syndrome is caused due to the excessive glomerular permeability to plasma proteins and massive proteinuria.

Causes

Nephrotic syndrome is caused due to damage in the clusters of tiny blood vessels (glomeruli) of the kidneys. There are many possible conditions that can cause damage to tiny blood vessels (glomeruli) of the kidneys and can lead to Nephrotic syndrome, such as

  • Membranous nephropathy – It is caused due to thickening of the glomerular basement membrane which helps to filter waste and extra fluid from the body.
  • Diabetic kidney disease – It is caused due to high levels of sugar in the blood that lead to damage in the nephrons in the kidney.
  • Focal segmental glomerulosclerosis – It is caused due to scarring of some of the glomeruli.
  • Systemic lupus erythematosus – It is an inflammatory condition which is also known as lupus nephritis.
  • Membranous nephropathy – It is caused due to the thickening in the membrane of glomeruli.
  • Amyloidosis – It is caused due to accumulation of the amyloid proteins in the organs.

Pathophysiology

  • Increase in the level of renal catabolism and inadequate hepatic synthesis can lead to hypoalbuminemia (low levels of albumin in the blood). The loss of colloid osmotic pressure in the blood and accumulation of fluid in the interstitial tissues can cause generalised edema.
  • The Glomerular podocytes get affected by several conditions which further lead to Nephrotic syndrome.
  • The Glomerular endothelial cells have several functions and filtration is also one of its as it works to trap the plasma proteins in the bloodstream when the function of filtration hampers it leading to proteinuria.

Nephrotic syndrome is characterized due to albuminuria with hyperlipidemia which further associates with generalized edema, High levels of protein in the urine, Low levels of protein in the blood. In the Ayurvedic approach the feature of this condition can be correlated with one of the types of Prameha (Urinary disorders) which is Sandrameha. According to Sandrameha, this condition is caused due to Tridosha predominantly Kapha dosha. In this condition there is aggravation of Kapha dosha along with imbalance of rasa dhatu (plasma/lymph fluid), mutravaha srotas (urinary system), ojas (bodily strength), udakavaha srotas (channels of the body which helps to regulate water). The patient suffering from this condition should avoid the intake of all kind of citrus fruits, green papaya, custard apple, tomato, eggplant, garlic, all green leafy vegetables, red chilies, green chilies, crab, lean beef pork, whole milk and cream, cottage, curd and all packed foods. Some of the yoga exercises are very helpful in maintaining kidney health such as Kapalbhati pranayama, Dhanurasana, Gomukhasana and various others.

Excessive intake of Nidanaggravates Tridosha predominantly Kapha dosha. The imbalance Kapha dosha starts circulation through the body and vitiates Medas (Marrow), Kleda (Body fluid), Mamsa (Muscle tissue). Further it also vitiates body fluids and transforms into urine then vitiated Kapha dosha reaches Mutravaha Srotas (Urinary system) and hampers its function.
