INTRODUCTION
In Intestinal Tuberculosis a Mycobacterium tuberculosis organism affects the lining of the intestines. It is of two types generally that are primary and secondary. The primary Intestinal Tuberculosis is not associated with any of the diseases but the secondary one is always associated with the pulmonary Tuberculosis. The prevalence of this disease in the patients of pulmonary Tuberculosis is more than the other those not associated with pulmonary tuberculosis. In this condition there is the deposition of the tubercle in the walls of the intestines that shows almost the same symptoms to that of crohn’s disease and very difficult to do the differential diagnosis. The commonly affected part of the intestines are the ileocecal region. Further we are going to discuss the causes and symptoms of Intestinal Tuberculosis.
TYPES OF INTESTINAL TUBERCULOSIS
The Intestinal or Abdominal Tuberculosis has three Variant Types Which are as Follows:
- Ulcerative intestinal tuberculosis: This type of intestinal tuberculosis includes the multiple ulcers that are situated superficially and are restricted up to the epithelial surface. This type of the disease is one of the most active forms.
- Hypertrophic intestinal tuberculosis: In this type the intestinal walls get thicken and leads to scarring and later on fibrosis. The appearance of the intestinal wall is rigid and the area seems to be like that of carcinoma.
- Ulcer Hypertrophic intestinal tuberculosis: The combination of hypertrophic ad ulcerative one is known as ulcer hypertrophic intestinal tuberculosis.
CAUSES OF INTESTINAL TUBERCULOSIS
As we discussed earlier, the main cause of Intestinal Tuberculosis is the bacteria Mycobacterium Tuberculosis. Other than that there is no other cause of Intestinal Tuberculosis. As we read earlier this condition is of two types primary and secondary, the primary one occurred due to administering the food which is infected and the secondary one occurs by the previous presence of tubercular infection in the body and people get affected with the intestinal tuberculosis by having infection via blood stream.
Common Pathway of Spread of Intestinal Tuberculosis
- Hematogenous spread
- Spread via contagious lymph nodes
- Infected sputum ingestion
- Spread via contagious fallopian tubes
SYMPTOMS OF INTESTINAL TUBERCULOSIS
As we studied earlier this condition has symptoms just like crohn’s disease, let’s see below what are the symptoms a patient may face
- Diarrhea
- Abdominal pain
- Fever
- Alternate constipation
- Bowel obstruction
- Abdominal distention
- Weight loss
- Malabsorption
- Fatigue
- Night sweats
- Fistula
DIAGNOSIS OF INTESTINAL TUBERCULOSIS
- Blood tests – that will detect the high ESR levels and low Hb levels.
- Biopsy
- Tuberculin skin test – a standardized test for mycobacterium tuberculosis organism
- Colonoscopy
- Endoscopy
- Radiography – barium enema, USG, CT – Scan of whole abdomen
AYURVEDIC VIEW OF INTESTINAL TUBERCULOSIS
As per ayurveda there is no direct correlation of intestinal tuberculosis but we can consider it as associated with kshaya roga. In kshaya roga there is depletion of immunity and when the immunity of the person depletes, the one is more susceptible to any kind of infections. As per ayurveda infections are correlated to agantuja karan, where the cause of disease is some external factors. Same like that happens in Intestinal Tuberculosis, the bacteria mycobacterium tuberculosis affects the intestines and leads to various symptoms such as Diarrhoea, Abdominal pain, Fever, Alternate constipation, Bowel obstruction, Abdominal distention, Weight loss, Malabsorption, Fatigue, Night sweats, Fistula and various other gastric issues. In continuity we will discuss the ayurvedic treatment for intestinal tuberculosis with herbal remedies.
