Vasculitic Neuropathy

Introduction

Vasculitic neuropathy is the part of systemic vasculitis but it also can be present as a non systemic vasculitic neuropathy. This condition is termed as peripheral neuropathic disease. In this condition there is damage to the vessels that supply blood to the nerves. There are three types of vasculitic neuropathy majorly and certain subtypes as well. Vasculitides are a diverse group of diseases clinically. The histopathological signature includes the centred inflammation of blood vessels which at last result in the vascular damage and ischaemic injury to the affected tissues. This can be caused by drugs, infections, cancers but the diagnostic workup never reveals any trigger in most of the patients. The autoimmune mechanisms are active in almost all types of vasculitis, except for the ones which are caused due to the direct infection on the vessel walls. Most of the vasculitides are systemic and involve multiple organs and tissues. But as mentioned before vasculitic neuropathy can also occur without systemic vasculitis.

Causes of vasculitic neuropathy

  • Vasculitic neuropathy can be due to many reasons. In 60% of the cases, uncontrolled diabetes is responsible for vasculitic neuropathy. In the remaining 40% the condition may arise due to the immunocompromised conditions like HIV, kidney disease, autoimmune diseases, infectious diseases, excessive alcohol intake and nutritional deficiencies.
  • The condition may persist as a hereditary disorder, may have a history of physical trauma and other causes.

Types of vasculitic neuropathy The Types of vasculitic neuropathy include the following

  • Primary systemic vasculitic neuropathy
  • Vasculitic neuropathy secondary to other disease

Primary systemic vasculitic neuropathy Some patients who are suffering from systemic vasculitis, will have their multiorgan disease spread to the peripheral nervous system. This is called primary vasculitic neuropathy. Some examples of systemic vasculitic disease are IgA vasculitis, hypocomplementemic urticarial vasculitis, polyarteritis nodosa (PAN) and many others.

Vasculitic neuropathy secondary to other disease

Some patients with non vasculitic systemic disease or some other illness such as infection or any type of malignancy or cancer gradually develop vasculitic neuropathy as a direct consequence of the former illness. This is called secondary vasculitic neuropathy. Conditions like connective tissue diseases like systemic lupus erythematosus, infectious diseases like hepatitis B, hepatitis C, malignancy etc. are the examples of vasculitic neuropathy secondary to other diseases.

Non- systemic vasculitic neuropathy

Non- systemic vasculitic neuropathy means there is no systemic illness, yet the evidence of a vasculitic neuropathy exists. This is a single organ problem and in this nerve biopsy is generally done to rule out the presence of this very type of vasculitic neuropathy. The non systemic vasculitic neuropathy has the following subtypes

  • Classical distal predominant Non systemic vasculitic Neuropathy
  • Wartenberg migratory sensory neuropathy
  • Post surgical inflammatory neuropathy
  • Diabetic radiculoplexus neuropathy
  • Neuralgic amyotrophy
  • Non systemic skin/nerve vasculitis.

Symptoms of vasculitic neuropathy

Generally the symptoms in this condition depend on the type and location of the nerve fibre involved. The condition generally causes sensory and motor dysfunction. The most common symptoms in this condition are as follows

  • Unusual sensations (Paresthesias)
  • Numbness in the peripheral areas
  • Weakness in the muscles of limbs either upper or lower
  • Burning sensation in the affected areas

Diagnosing vasculitic Neuropathy

  • The diagnosis depends upon whether the patient first presents with multiple symptoms pointing to systemic disorder. Or else the symptoms may suggest primary neuropathic complaints.
  • The diagnosis is also done on the basis of history, clinical examination and supporting lab investigations. The tests include electromyography with nerve conduction studies. Skin biopsies are also done to rule out cutaneous nerve innervation. Nerve and muscle biopsies are also done for histopathological evaluation.

Treatment of vasculitic Neuropathy

Vasculitic neuropathy’s treatment depends on the type. Acute exacerbations of symptoms can sometimes be treated with intravenous immune globulins and even with plasma exchange. The neuropathic pain due to vasculitic neuropathy is well managed by using anti seizure medications, antidepressants and analgesics. It also includes use of opiate drugs.

Ayurvedic aspect of vasculitic neuropathy

Vasculitic neuropathy involves aggravated vata dosha. The aggravated vata dosha is due to the accumulation of ama dosha which has similar characteristics or properties as vata dosha. Then this vata dosha in turn results in the provoking aggravation of Pitta Dosha. Due to these aggravated dosas the patient experiences various types of symptoms like tingling sensation along with pricking pain. These two symptoms are the typical characteristic of pains which are due to aggravated vata dosha. On the other hand the pin is followed by a burning sensation. This burning sensation depicts the involvement of pitta dosha.

Initially it all starts with ama accumulation and blockage of srotas. When the srotas are blocked, the vata dosha is aggravated or its gati or direction of flow is disturbed. Then it vitiates everything that comes in its path like pitta dosha in this situation.

The chikitsa in vasculitic neuropathy is done with herbs that help in the pacification of vata and pitta dosha. Herbs like haridra, shallaki, manjistha have very great results in this condition.
