Ayurvedic Aspect of Encapsulating Peritoneal Sclerosis

ABSTRACT

Encapsulating Peritoneal Sclerosis (EPS) is an uncommon cause of intestinal obstruction that is characterised by fibrotic encapsulation of the colon as a result of a progressive intra-abdominal inflammatory process. The main symptoms of encapsulating peritoneal sclerosis include inflammatory parameters, gastrointestinal obstruction, and macroscopic abnormalities. By the International Society for Peritoneal Dialysis as a syndrome characterised by symptoms of intestinal obstruction caused by diffusely thickened peritoneum adhesions. Now, we discuss Encapsulating Peritoneal Sclerosis in detail.

INTRODUCTION

Encapsulating Peritoneal Sclerosis is a rare but severe peritoneal dialysis complication. This condition primarily affects people who have been on peritoneal dialysis for a long time. Encapsulating Peritoneal Sclerosis is also known as abdominal cocoon syndrome, calcific peritonitis, peritoneal fibrosis, peritoneal sclerosis, and sclerosing encapsulating peritonitis. It is classified into four stages: pre-eps, inflammatory illness, encapsulation, and chronic ileus. Early detection of this condition is critical for a better prognosis.

Causes of Encapsulating peritoneal sclerosis

CLASSIFICATION OF ENCAPSULATING PERITONEAL SCLEROSIS

The clinical stages of encapsulating peritoneal sclerosis are as follows

  • Stage 1: Pre-EPS stage: The patient is asymptomatic with minimal ascites and no inflammation at this stage.
  • Stage 2: Inflammatory Disease: Patients in this stage of encapsulating peritoneal sclerosis are symptomatic. The main symptoms of this stage are nausea and diarrhoea.
  • Stage 3: Encapsulation: Inflammation can occur at this period. Inflammation can range in severity from moderate to severe.
  • Stage 4: Chronic ileus: Patients have absolute intestinal obstruction due to thickening of the encapsulating fibrous cocoon. At this point, there is little inflammation.

AYURVEDIC ASPECT OF ENCAPSULATING PERITONEAL SCLEROSIS

According to Ayurveda, encapsulating peritoneal sclerosis is associated with anna vaha sroto dushti, which aggravates vata dosha. Abdominal pain, intestinal blockage, and constipation come from vata aggravation. Some ayurvedic remedies, such as snehana, swedana, and shodhana, can help balance vata. Muscle fat, ghee, oil, and bone marrow are administered to the patient in snehana to balance the doshas. Following snehan, patients are offered fomentation therapies such as nadi sweda, prastara sveda, and sankara sveda, among others. The herbs varun, bhumiamla, gokshur, punarnava, and triphala are beneficial in the treatment of encapsulating peritoneal sclerosis.
