ABSTRACT

West syndrome in the very first phase of disease is a combination of axial spasms, hypsarrhythmia and psychomotor. Variations may occur in the symptoms depending on the patient’s age and aetiology of disease. 10-20% cases are due to unknown reasons. Another term for west syndrome nowadays refers to infantile spasms syndrome.It typically affects infant age more than 2 years.Increasing no of genes and signal pathways is very crucial in pathogenesis. Hormonal therapy is used in the treatment with other pharmaceutical drugs. As per ayurveda this condition can be correlated with the Apasmara along with the delaying development of mental state. 

INTRODUCTION

West syndrome is the uncommon rare type of epileptic syndrome that is mostly seen in children of age 2years. It is named after the English physician William James West which describes the infantile spasm firstly. West described the typical symptom is the bowing and relaxing alternatively in intervals of a few seconds and repeated 10-20 times each time. The type of seizures depend on the involvement of the muscle group. Most of the children suffering from this disease face learning disabilities which may be mild, moderate and severe.

  • Interictal electroencephalogram
  • Hypsarrhythmia
  • Mental retardation

WHAT ARE THE CAUSES?

It is classified into prenatal, perinatal and postnatal groups.

  • Prenatal disorders : Hydranencephaly, migrational disorders,  congenital central nervous system infection
  • Perinatal : hypoxic ischemic encephalopathy, meningitis, trauma, intracranial haemorrhage.
  • Postnatal: hyperglycemia, phenylketonuria, biotinidase, lysosomal disorder etc.

  • Williams beuren syndrome

PATHOPHYSIOLOGY

Based on some theories it is believed that corticotropin releasing hormone (neurotransmitter) acts on the pituitary to release ACTH(adrenocorticotropic hormone)  in high amounts. ACTH and glucocorticoids give a feedback mechanism to the CRH that causes neuronal excitability and seizures.

Brain activity can be measured from different devices some of the following are

  • EEG(Electroencephalography)
  • Computed tomography(CT)
  • Magnetic Resonance Imaging(MRI)

DIFFERENTIAL DIAGNOSIS

West syndrome may sometimes mimic the symptom of

  1. Benign spasm of infancy
  2. Myoclonic epilepsy of infancy

AYURVEDIC VIEW

It is considered to be the age dependent factor which affects the brain and causes damage to it with time. In most of the patients in infantile spasm development of mental growth is delayed. It is correlated with the disease Apsamara in which involvement of vata dosha can be seen. West syndrome is considered to be vataj apasmara because of frequent attacks of seizure.

In apasmara it is stated that the dosha are stated in leena avastha in hrudya, Manovahstrotas and Indriya and buddhi when they are exposed to causes like kama(desires), krodha(anger), bhaya(fear). In diseases like apasamra where balakshmata (immunocompromised) can be seen. If proper treatment is not taken then there is a chance of the relapsing of disease which is not benefitted to the patient. Ayurvedic medicines treat patients in that way so that these doshas cannot get vitiated and also proper recommendations of diet were given only in Ayurveda.

Dosage: 2 tsp twice daily with plain water after meals.

4. KUMAR KALYAN RAS

It is typically given to children to enhance immunity, resist respiratory infections, also helps in jwara , kasha and swasha etc. This formulation consists of the following ingredients such as: Ras sindoor, Moti pishti, Swarna makshik bhasma etc. This formulation consists of purified metal components so it contains good values. It can be used as immunostimulatory because it helps to develop nonspecific immunity and its rasayana effect improves memory.  This formulation is beneficial in various organs like: liver, lungs and brains etc.

Dosage: 1 tablet daily to be chewed with plain water

5. BRAHMI GRIHIT

It is considered to be beneficial in the Treatment of Brain Diseases. It is seen when we use Brahmi(Bacopa monnieri) for prolonged time it causes elevation of glutamic acid and also increases gamma aminobutyric acid which is helpful in the process of learning.

Dosage: 3 drops twice daily , if the child is above 7 years then half teaspoon twice with warm milk.

6. ANU TAILAM

It contains 29 ingredients some of them are listed:til tail, devadar, ushir, Daruharidra, Bala etc. It  is very beneficial for the prevention of many diseases such as: baldness, facial palsy, loss of memory, nose congestion etc. Therefore it can be used and patients benefit from it in their disease.

Dosage: Children above 7 years can take two drops daily in each nostril.

CONCLUSION

West syndrome is difficult to treat when compared to other epileptic diseases. If delayed in diagnosis it can cause intellectual disability and even fatal in some cases. Therefore diagnosis should be done as soon as possible.  In western medicine treatment can be done but sometimes it can’t control seizures and it has side effects. For avoiding these side effects patients can choose ayurvedic medicine and benefit. Ayurvedic drugs are not like synthetic drugs they can treat well.
