Abstract

Colles fracture is named after Abraham Colles who described  fracture of distal radius for the first time in Dublin at Royal College of Surgeons. It is one of  the most common fractures that are presented in the orthopaedics department. This is a fracture at the distal end of the radius at the cortico-cancellous junction (about 2cm from the distal articular surface), in adults, with typical dorsal replacement. It usually results from a fall on an out-stretched hand. In the following text we shall look in some detail about colles fracture and its ayurvedic management. Let’s begin !!

Introduction

Colles fracture is defined as a distal radius fracture, with dorsal angulation, dorsal comminution, dorsal displacement, radial shortening and associated fracture of ulnar styloid.’Colles fracture’ is the term used for distal fractures with dorsal angulation. These distal radius fractures are usually caused by falling on an outstretched hand with the wrist placed in dorsiflexion, causing strain on the volar aspect of the wrist, which results in the fracture to stretch posteriorly. The fracture line runs transversely at the corticocancellous junction. In majority of the cases, dorsal displacements occur; although in a few cases it may be crack fracture without displacement.

Aetiology

It is the commonest fracture in people above forty years of age and is particularly in women because of postmenopaisal osteoporosis. It can usually happen during contact sports in athletes , skiing, horse riding, motorcycle accidents, falls from a height. With increasing knowledge/awareness about osteoporosis has led to these injuries being known as fragility fractures and therefore managing osteoporosis with treating the fracture has become a standard part.

Clinical features

  1. Dinner fork deformity
  2. Dorsal wrist pain
  3. Increased angulation of the distal radius
  4. Inability to grasp an object
  5. Swelling of the wrist
  6. History of trauma or fall

Diagnosis

  1. A normal or plain X ray usually represents the fracture.
  2. A CT scan can be used to detect occult fracture.
  3. An MRI can assess ligamentous and soft tissue extent of the injury. 

Management

  1. For non-displaced fracture closed reduction under general anaesthesia is done and immobilisation by below elbow cast for at least six weeks is sufficient.
  2. For displaced fracture ‘Open reduction and Internal Fixation.’
  3. Analgesics are given to alleviate pain.
  4. Calcium supplements are also used.

1. Lakshadi Guggul

It is the formulation which is made up of Laksha which is the resin from female insect lac. It is a formulation made with the goodness of Asthishrinkhala (Cissus quadrangularis), Laksha (Laccifer lacca), Arjuna (Terminalia arjuna), Ashwagandha (Withania somnifera), Nagbala (Grewia populifolia), Shuddha guggulu (purified Commiphora mukul). Praval pishti and mukta are natural supplements of calcium which are derived from the pearls, corals etc. thus are beneficial in early healing or adhesion of the fractured bone. 4. Dashmoola Capsules

It contains Bilva (Aegle marmelos), shyonak (Oroxylum indicum), patla (Stereospermum suaveolens), gambhari (Gmelina arborea), agnimantha (Clerodendrum phlomidis), gokshur (Tribulus terrestris), brihati (Solanum indicum), kantkari (Solanum surattense), prishniparni (Uraria picta) , shalparni (Desmodium gangeticum). Dashmoola is basically indicated in arthritis and antiinflammatory conditions. And it is used as an analgesic. So it becomes important in the case of fracture.
