Abstract

The name of this condition is Leptomeningeal Enhancement. In this article we will discuss its details along with its Ayurvedic management.
Introduction
Leptomeningeal enhancement is a non-specific imaging feature. Leptomeningeal enhancement or disease is also termed as a late stage complication of systemic cancers. The reason for this is the multifocal metastases to the leptomeninges. This lepto meninges consist of pia mater, arachnoid mater and subarachnoid space. The commonest neoplastic etiologies of leptomeningeal disease are breast cancer, lung cancer, gastro intestinal, melanoma and primary central nervous system cancers like medulloblastoma, ependymoma, pineoblastoma, primitive neuroectodermal or primary CNS lymphoma. The conditions like non hodgkin’s lymphoma, leukaemia and multiple myeloma can also cause this condition. Even after targeted radiation and chemotherapy, survival remains poor. But in case of breast cancer the best prognosis is observed, that is 13-25% survival is noted.
Epidemiology
- The data suggest that 1%-8% of the cancer patients are being diagnosed with leptomeningeal disease of enlargement. This means around 110,000 cases per year.
- This condition is found in 1-5% of patients with solid tumours.
- Also in 1-2% of patients with primary brain tumours

- It is a carcinomatous meningitis
- Mostly exist as a devastating complication of advanced cancer
- Simultaneous involvement of multiple areas of craniospinal axis
- Diagnosis is generally made by analysis of cerebrospinal fluid

Cause of Leptomeningeal Enhancement
As mentioned before this condition generally coexists with advanced stages of cancer where metastases have taken place. Firstly the metastasis happens in the fluid surrounding the brain tissue. Then through this tissue it spreads to meninges resulting in their enhancement.
Symptoms of Leptomeningeal Enhancement
The generally observed symptoms in patients with leptomeningeal enhancement are mostly confused with other diseased conditions. The clinical manifestations are as follows
- Headache
- Altered mental status
- Cerebellar dysfunction
- Cranial neuropathies
- Cauda equina syndrome
- Seizures
- Plateau waves (Consist of sudden and rapid elevation of intracranial pressure)
Diagnosis of Leptomeningeal Enhancement
The neuroimaging studies are generally done for these conditions. Following diagnostic techniques are commonly used
- Magnetic resonance imaging of brain
- Magnetic resonance imaging of spine
- Testing of cerebrospinal fluid for tumour markers, flow cytometry, cytology, circulating tumour cells or DNA
Treatment of Leptomeningeal Enhancement

Ayurvedic view point about Leptomeningeal enhancement
- Leptomeningeal enhancement as mentioned above is the result of metastatic cancer mostly. Ayurveda suggests that cancer is the result of all three dosha aggravations along with dhatu dushti as well. As this is the case of metastasis obviously there is dosha involvement here as well. In this, there is tridosha aggravation, predominantly vata dosha and kapha dosha. The symptoms in this condition like cerebral dysfunction which is due to vata dosha, headaches due to kapha dosha, seizures due to vata and kapha dosha explains clear cut dominance of these two dosha.
- Leptomeningeal enhancement is also having symptoms like pains in various parts of the body which at last is the result of vitiated vata dosha in the body, disruption of blood brain barrier, visual disturbances is also the result of vitiated vata dosha. Ischemia in vessels (if involved) is due to both vata and kapha dosha vitiation.
- In this case a very precise treatment is to be given. A combination of herbs and herbal formulations that can work in the pacification of vata and kapha dosha. Also can help in the enhancement of immunity of a person so that the body’s capacity is raised to fight various types of unwanted processes going on inside the body. Product Description
1. NEUROGENIE CAPSULES
Neurogenie capsules, as its name suggests are the genie for the neurological system. The formulation has herbs like brahmi (Bacopa monnieri) and ashwagandha (Withania somnifera).
